| CDHS issues the following Certificates: |
Certificate of Attendance - indicates a specific person attended and participated in an instructor-led workshop, teleconference, or iLinc online course. These certificates are issued to participants following their successful completion of the course. Certificate of Completion - indicates that a specific person has completed the two-day train-the-trainer course for mandated reporters. This certificate indicates that the recipient attended the revised, OCFS-approved mandated reporter course and can train instructor-led workshops for other mandated reporters who are not licensed by the New York State Education Department. This certificate IN NO WAY gives the recipient the authority to sign State Education documents. Only the New York State Education Department can register and certify providers. To become a certified provider contact the New York State Education Department at www.op.nysed.gov. Certification of Completion - this is a legal document that can only be signed by a New York State Education Department certified provider. CDHS is a New York State Education Department certified provider. Currently the following professionals require certification of completion: chiropractors, dentists, dental anesthesiologists, dental hygienists, certified dental assistants, physicians, physician assistants, mental health practitioners (creative art therapists, marriage and family therapists, mental health counselors , and psychoanalysts), RNs, LPNs, nurse practitioners, optometrists, podiatrists, psychologists, and social workers.
